The Registered Nurse - Clinical (RN) at Open Door Community Health Centers (ODCHC) provides general nursing care to patients in the clinic and over the phone. The RN plays an essential role in providing pt access through visits using protocols and transitioning to provider whenever able or needed. Within the Care Team, the RN works as a point person for daily functions and triage.
Compensation Range:
$40.00-$46.00
The hourly pay for this position ranges from $40.00 to $46.00. All new hires will begin at $40.00 per hour not including education differentials below.
$2.00/HR additional for BSN
$3.50/HR additional for MSN
Wage differentials do not compound.
Hmong and Spanish speakers who successfully pass a language exam will receive an additional .75 hourly differential to their wage.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Responsible for phone triage assessment and/or in-person triage for patients with urgent needs.
- Schedules patients after appropriate triage.
- Manages 4-6 patient visits per day of various types- immunizations, education, co-visits, and the schedule will be determined with the RN Clinic Manager.
- Explains procedures and treatments to patients to gain cooperation, understanding, and allay apprehension.
- Responsible for patient assessment based on chief complaint and treatment as outlined by Nursing Protocols or provider direction.
- Observes patient, records, significant conditions, and reactions, and notifies supervisor or Provider of patient's condition and reaction to drugs, treatments, and significant incidents.
- Communicates directly with clinic providers regarding patient assessment and needs; transitions patient to the provider as needed.
- Utilizes Situation, Background, Assessment, Recommendation (SBAR) warm hand-off method.
- Recommends and/or administers medications and vaccines as ordered by medical provider or by protocol.
- Responsible for coding each visit accurately.
- Accurately records patient history, exam notes, medication history, ongoing care, and referrals in employee medical record (EMR) according to ODCHC Protocols.
- Manages tasks in their inbox and appropriate tasks in their assigned providers’ inboxes; ensures team inbox tasks are completed in a timely manner according to team role and scope.
- Assists in Chronic Care management, such as outreach calls, care coordination, etc.
- Assists and/or facilitates routine daily huddles within the team
- Provides oversite to the MA working in the Care Team as needed for safe vaccine delivery.
- Maintains awareness of the comfort and safety needs of the patient.
- Responds to medically urgent situations based upon nursing standards, policies, procedures, and protocol.
- Prepares and maintains a wide variety of records and reports.
- Teaches patients, families, and staff care, prevention, and treatment enhancement techniques.
- Provides direct clinical oversight for Medical Assistants (MA) and Licensed Vocational Nurses (LVN) assigned to Care Team.
- Attend and participates in meetings and Quality Improvement activities as required.
- Identifies problems related to patient services and makes recommendations for improvement.
- Participates in the evaluation of peers and support staff.
- Participates in Open Door Quality improvement activities, meetings, and initiatives.
- Performs other duties as identified and assigned by supervisor.

- Participates in staff meetings as required.
- Ability to exercise sound and responsible judgment.
- Strong computer skills with specific aptitude in Microsoft Office Suite and/or Electronic Medical Records (EMR).
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
- Ability to effectively represent ODCHC’s interests in the community and maintain effective working relationships among co-workers, public, private, and professional groups.
- Adheres to ODCHC’s policies and procedures.
EDUCATION AND EXPERIENCE:
- An Associate’s Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) is required.
- Previous clinical experience preferred.
CERTIFICATES, LICENSES, AND REGISTRATIONS:
- Current Registered Nurse license for the State of California
- Current Basic Life Support (BLS)/CPR Certification
- Successful completion of Triage Training modules, and competency sign-off within 90 days of hire.
SUPERVISORY RESPONSIBILITIES: The RN works as a member of the Care Team and my act as a clinical supervisor as needed for MAs and LVNs on the Care Team.
SUPERVISION AND SUPPORT: The RN reports directly to the RN - Clinic Manager. The RN works directly with providers and support staff assigned to their Care Team and across the clinic.
